EMS and CHC are pleased to provide the VPN program for our residents in Fontainebleau Towers and Raymond Desmarais Manor. The purpose of the program is to ensure seniors with frequent 911 calls are utilizing the most appropriate resources to them as they manage chronic diseases and ailments.
EMS Navigators will facilitate the healthcare of vulnerable residents by indentifying needs, advocate for residents by connecting them with the appropriate resources available, coordinate our residents treatment plnas to ensure follow through care and educate our residents about healthy choices as well as on going health manangement.
The goal is to provide a single contact person, through EMS so vulnerable residents can get the assistance needed before it is a 911 call.
