Important Information from the Landlord and Tenant Board
1. Consent to service by email must be in writing, although parties do not have to use this form.
2. Consent to service by email may be revoked at any time by giving notice in writing to the other person.
3. When serving a document by email, the name and telephone number of a person to contact must be included.
4. Make sure to keep a copy of this form for your records.
